M: Hello, doctor.

W: Hello, please take a seat. I have your test results here, and it's good news. The blood test came back clear. There is no indication of any digestive issues.

M: So then why do I feel so poorly all the time?

W: It's probably due to overwork and stress.

M: No, it can't be. I've always been working hard, but I've never felt stress. Other people suffering complain about that, but I don't. It must be something else.

W: What you have just described is a common sentiment. Many people who suffer from stress fail to recognise it. You told me you often work long into the night, right?

M: Yes, most days in fact. But I've been doing that for about 20 years now.

W: That doesn't matter. You could have been suffering from stress for 20 years without knowing it, and now it's catching up to you.

M: But what about my feeling tired all the time and not being able to sleep well at night?

W: Those are common consequences of stress. And if you don't sleep well, then of course you will feel fatigued. I'm going to prescribe some special sleeping pills for you. They have a soft, gentle effect and are made from natural ingredients, so your stomach should tolerate them fine, and there shouldn't be any negative side effects. Take one with your dinner and come see me after a month. If there is no improvement, I'll give you something stronger.

M: Thank you, doctor.

W: That's not all. You should try and work less. Is there any way you can decrease your workload?

M: Um, I'd have to think about it. I'm a restaurant manager and this industry is very competitive. There are many things to keep track of and stay on top of.

W: I recommend you think about delegating some responsibilities to someone else. I'm not asking you to retire just to slow down a bit. It's for your own health.

Doctor 和 patient 进行 greeting 后,直接 present 了 test results,明确 blood test 显示 clear, ruling out digestive issues 这个可能性

Patient 表达 confusion 关于为什么 feel poorly, doctor 给出 initial diagnosis 认为是 overwork and stress 导致的,但 patient immediately denies 这个 possibility

这个 意群 introduces 文章的 central conflict,即 patient's denial of stress 和 doctor's diagnosis 之间的 tension, directly addresses 主旨中的 despite the patient's denial 这个 key element

Doctor 针对 patient's denial 进行 detailed explanation,指出 many people fail to recognize stress,然后 inquire about working habits, patient confirms long working hours over 20 years, doctor 强调 duration doesn't matter 并说明 chronic stress 的 cumulative effect

What you have just described is a common sentiment 和 suffering from stress for 20 years without knowing it 来 establish 长期 unnoticed stress 的 medical validity

Patient 提出 specific symptoms 包括 feeling tired 和 sleep problems, doctor 给出 comprehensive explanation 说这些是 common consequences of stress,然后 prescribe sleeping pills 并详细说明 medication properties 和 follow-up plan

这个 意群 demonstrates 主旨中 manifest physically 的具体体现,通过 tired all the time 和 not being able to sleep well 来 illustrate,同时 presents 主旨里的 prescribes sleeping pills 这个 treatment action

Doctor 用 That's not all 来 introduce additional advice,建议 decrease workload, patient 表达 difficulty 因为 competitive industry, doctor 进一步 recommend delegating responsibilities 并 emphasize 这是 for your own health

It's for your own health 来 emphasize treatment 的 preventive dimension

MAIN IDEA ·

Patient's complaints 患者的抱怨直接 reveal what needs to be addressed 揭示了需要解决的内容。

在 consultation dialogues 咨询类对话中,the authority figure's judgment 权威人物的判断构成了 the main message 主要信息。Doctor's conclusion 医生的结论不仅 identifies the cause 识别了原因,还 connects symptoms to root problem 将症状与根本问题联系起来,这就是 the thesis 论点。

在 problem-solution dialogues 问题-解决方案对话中,the solution 解决方案必须 match the diagnosis 匹配诊断。 但她的 advice to reduce work 减少工作的建议 confirms 确认了 stress from overwork 过度工作导致的压力才是 the diagnosed problem 被诊断的问题。

Single elements 单个元素只是 supporting details 支持细节,only the complete narrative 只有完整叙事才能 capture the main idea 捕捉主旨。

MAIN IDEA ·

A doctor diagnoses a patient's fatigue and sleep problems as stress-related despite the patient's denial. She prescribes sleeping pills and advises reducing workload, explaining that chronic overwork can cause unrecognized stress symptoms that eventually manifest physically.
WORD INFERENCE ·

平行结构揭示 tolerate 意味着 no negative reactions,是 acceptance without adverse effects。

    P3

患者在 第3段 使用反问句 'So then why do I feel so poorly all the time?'

So then why Then how But why So what Then what about How come Why on earth How is it that

比如面对 unexpected news 时,不直接说 'I don't understand',而是问 'Then how could this happen?'。 So/Then + question word (why/how/what) + strong feeling/contradiction?',例如:'So then how am I supposed to fix this?' 或 'Then why did nobody tell me earlier?'

    P5 P7

患者在 第5段 和 第7段 中反复强调 'I've always been working hard' 和 'I've been doing that for about 20 years now',这种 repetition 体现了他的 denial 和 defensive attitude。 这个修辞手法凸显了患者的 resistance to diagnosis,恰恰印证了医生后来说的 'Many people who suffer from stress fail to recognise it',深化了文章关于 unrecognized stress 的 theme。

I've always been I've been doing I always I've never every time for years all the time consistently

I've always/been + [action/state] + time expression',然后在后续 turn 用 'I've been doing that/this for + duration'。 I've always paid my bills on time.' 然后 'I've been managing my finances carefully for years.'

    P5 P13

患者在 第5段 使用 'No, it can't be' 和在 第13段 使用 'Um' 这些 discourse markers 来表达他的 emotional state 和 hesitation。'No, it can't be' 中的 'No' 是强烈的 rejection marker,显示 immediate resistance;'Um' 则显示 uncertainty 和 reluctance to commit。

No Um Well Oh Ah You know I mean Actually So But

在表达 strong disagreement 时用 'No' 开头,在 hesitate 或 need thinking time 时用 'Um' 或 'Well'。 No/But + [statement of disagreement]' 表示 rejection;'Um/Well + [hesitant response]' 表示 uncertainty。 No, that's not what I meant.' 或 'Um, I'm not sure about that approach.'

    P12

    在 第12段 医生说 'That's not all' 之后直接给出 advice,省略了 transition 如 'There's something else I need to tell you' 或 'Additionally'。

比如连续给 instructions 时,不必每次都说 'You should' 或 'I want you to'。 例如:'Take this medicine. [省略 Also/And] Come back next week. [省略 Then] We'll check your progress.' 这比每句加 'You should' 或 'I need you to' 更 natural 和 efficient。

IMPLIED MEANING ·

    P5
I've always been working hard, but I've never felt stress.

转折类 but however yet nevertheless nonetheless though although

Patient 说"I've always been working hard, but I've never felt stress",这个 but 后面的 absolute denial 其实 reveals 他对 stress 这个 diagnosis 的 resistance。医生 later 就 point out"Many people who suffer from stress fail to recognise it",directly addresses 这个 implication。 Without 这个 denial,整个 dialogue 就失去了 its core conflict - 即 patient's lack of awareness versus doctor's professional insight 的 contrast。

    P8
That doesn't matter. You could have been suffering from stress for 20 years without knowing it

That doesn't matter That's irrelevant That's beside the point Regardless Irrespective of Even so That said

Doctor 说"That doesn't matter"来 dismiss patient 的"20 years"argument,这个 dismissal carries 重要 implication:chronic problems 可以 silently accumulate 而不被 noticed。接下来"it's catching up to you"这个 phrase further emphasizes 这个 hidden consequence。这个 言外之意 directly supports 文章主旨中的"chronic overwork can cause unrecognized stress symptoms that eventually manifest physically"。它 breaks down patient 最后一道 defense mechanism - 即"I've been fine so far"的 logic,为 doctor's prescription 和 lifestyle advice 铺平道路。Without 这个 reframing, patient 会 continue 用 past experience 来 justify current behavior。

    P12
That's not all. You should try and work less.

That's not all Furthermore Moreover In addition What's more Besides Additionally

Doctor 用"That's not all"来 introduce workload reduction advice,这个 phrase implicitly tells patient:pills alone won't fix your problem。这个 implication 是 crucial 因为它 shifts responsibility from passive treatment to active lifestyle modification。 The 言外之意 makes clear 医生的 holistic approach - 她 not just treating symptoms (sleep problems) but addressing root cause (overwork and stress)。 I'm not asking you to retire just to slow down"提供了 logical foundation。

    P14
I'm not asking you to retire just to slow down a bit. It's for your own health.

It's for your own health It's for your own good This is for your benefit It's in your best interest For your own sake For your wellbeing This is important for you

Doctor 用"It's for your own health"来 conclude 她的 workload reduction advice,这个 explicit justification carries strong implication:if you don't make this change,后果 will be serious。这不是 casual suggestion,而是 medical necessity disguised as gentle advice。 她用 understatement ("just slow down a bit") combined with explicit health reminder 来 balance between not alarming patient 和 making sure he understands gravity。这个 closing statement ties together entire dialogue:from initial denial, through diagnosis,to actionable recommendation, all building toward 这个 implicit message that patient's current lifestyle trajectory is unsustainable and health-threatening。

掌握这种结构后,学生可以迁移到类似的 "What do you think..."、"What does he believe..."

  • What does the man say about the industry he is engaged in?
