Reform and Medical Costs

Americans are deeply concerned about the relentless rise in health care costs and health insurance premiums. They need to know if reform will help solve the problem. The answer is that no one has an easy fix for rising medical costs. The fundamental fix—reshaping how care is delivered and how doctors are paid in a wasteful, abnormal system—is likely to be achieved only through trial and error and incremental(渐进的) gains.

The good news is that a bill just approved by the House and a bill approved by the Senate Finance Committee would implement or test many reforms that should help slow the rise in medical costs over the long term. As a report in The New England Journal of Medicine concluded, "Pretty much every proposed innovation found in the health policy literature these days is contained in these measures."

Medical spending, which typically rises faster than wages and the overall economy, is propelled by two things: the high prices charged for medical services in this country and the volume of unnecessary care delivered by doctors and hospitals, which often perform a lot more tests and treatments than a patient really needs.

Here are some of the important proposals in the House and Senate bills to try to address those problems, and why it is hard to know how well they will work.

Both bills would reduce the rate of growth in annual Medicare payments to hospitals, nursing homes and other providers by amounts comparable to the productivity savings routinely made in other industries with the help of new technologies and new ways to organize work. This proposal could save Medicare more than $100 billion over the next decade. If private plans demanded similar productivity savings from providers, and refused to let providers shift additional costs to them, the savings could be much larger. Critics say Congress will give in to lobbyists and let inefficient providers off the hook(放过). That is far less likely to happen if Congress also adopts strong "pay-go" rules requiring that any increase in payments to providers be offset by new taxes or budget cuts.

The Senate Finance bill would impose an excise tax(消费税) on health insurance plans that cost more than $8,000 for an individual or $21,000 for a family. It would most likely cause insurers to redesign plans to fall beneath the threshold. Enrollees would have to pay more money for many services out of their own pockets, and that would encourage them to think twice about whether an expensive or redundant test was worth it. Economists project that most employers would shift money from expensive health benefits into wages. The House bill has no similar tax. The final legislation should.

Any doctor who has wrestled with multiple forms from different insurers, or patients who have tried to understand their own parade of statements, know that simplification ought to save money. When the health insurance industry was still cooperating in reform efforts, its trade group offered to provide standardized forms for automated processing. It estimated that step would save hundreds of billions of dollars over the next decade. The bills would lock that pledge into law.

The stimulus package provided money to convert the inefficient, paper-driven medical system to electronic records that can be easily viewed and transmitted. This requires open investments to help doctors convert. In time it should help restrain costs by eliminating redundant tests, preventing drug interactions, and helping doctors find the best treatments.

Virtually all experts agree that the fee-for-service system—doctors are rewarded for the quantity of care rather than its quality or effectiveness—is a primary reason that the cost of care is so high. Most agree that the solution is to push doctors to accept fixed payments to care for a particular illness or for a patient's needs over a year. No one knows how to make that happen quickly. The bills in both houses would start pilot projects within Medicare. They include such measures as accountable care organizations to take charge of a patient's needs with an eye on both cost and quality, and chronic disease management to make sure the seriously ill, who are responsible for the bulk of all health care costs, are treated properly. For the most part, these experiments rely on incentive payments to get doctors to try them.

Testing innovations do no good unless the good experiments are identified and expanded and the bad ones are dropped. The Senate bill would create an independent commission to monitor the pilot programs and recommend changes in Medicare's payment policies to urge providers to adopt reforms that work. The changes would have to be approved or rejected as a whole by Congress, making it hard for narrow-interest lobbies to bend lawmakers to their will.

The bills in both chambers would create health insurance exchanges on which small businesses and individuals could choose from an array of private plans and possibly a public option. All the plans would have to provide standard benefit packages that would be easy to compare. To get access to millions of new customers, insurers would have a strong incentive to lower their prices, perhaps by accepting slimmer profit margins or demanding better deals from providers.

The final legislation might throw a public plan into the competition, but thanks to the fierce opposition of the insurance industry and Republican critics, it might not save much money. The one in the House bill would have to negotiate rates with providers, rather than using Medicare rates, as many reformers wanted.

The president's stimulus package is pumping money into research to compare how well various treatments work. Is surgery, radiation or careful monitoring best for prostate(前列腺) cancer? Is the latest and most expensive cholesterol-lowering drug any better than its common competitors? The pending bills would spend additional money to accelerate this effort.

Critics have charged that this sensible idea would lead to rationing of care. (That would be true only if you believed that patients should have an unrestrained right to treatments proven to be inferior.) As a result, the bills do not require, as they should, that the results of these studies be used to set payment rates in Medicare.

Congress needs to find the courage to allow Medicare to pay preferentially for treatments proven to be superior. Sometimes the best treatment might be the most expensive. But overall, we suspect that spending would come down through elimination of a lot of unnecessary or even dangerous tests and treatments.

The House bill would authorize the secretary of health and human services to negotiate drug prices in Medicare and Medicaid. Some authoritative analysts doubt that the secretary would get better deals than private insurers already get. We believe negotiation could work. It does in other countries.

Missing from these bills is any serious attempt to rein in malpractice costs. Malpractice awards do drive up insurance premiums for doctors in high⁃risk specialties, and there is some evidence that doctors engage in "defensive medicine" by performing tests and treatments primarily to prove they are not negligent should they get sued.

New England Journal of Medicine的权威背书为改革措施的comprehensive性质背书。

每项措施都涉及cost-saving的具体机制(如reducing redundant tests、shifting costs to patients、standardizing forms),同时坦承political will和lobbying resistance的挑战,这种balanced argumentation使全文论证更具credibility,也与主旨中"成功依赖proper implementation和political will"的判断形成直接呼应。

It does in other countries"这一comparative argument增强说服力;第17段则点出这些法案中的重大缺失——没有serious attempt去rein in malpractice costs,并解释了defensive medicine如何推高成本。

MAIN IDEA ·

当作者用 no one knows、it is hard to、might、could、if 等 hedging language 时,说明主旨包含 conditional elements 或 acknowledge limitations。

议论文常通过 present and refute opposing views 来 strengthen 自己的 position。当文章用 critics say/charge、opposition、some doubt 引入 counter-arguments,然后用 but、however、that would be true only if 来 respond 时,这种 dialogic structure 帮助我们识别作者真正要 defend 的 position,因为作者 spend effort refuting 的往往是 threatens their main argument 的观点

主旨信号词 numerous innovative measures P2 should help slow P2 systematic changes P1 trial and error P1 political will P5 critics say P5 contained in these measures P2 we believe P15

MAIN IDEA ·

    整合三条线索:从 selection context 看,array 出现在 choose from 的语境中,暗示 multiple options;从 enumeration structure 看,文章列举了 private plans 和 public option,强调 diversity;从 phrase pattern 看,an array of 是表示'一系列'的固定搭配。

(Hypothetical Conditionals) That is far less likely to happen if less likely... if only if provided that unless depends on

    P7
Any doctor who has wrestled with multiple forms... or patients who have tried to understand their own parade of statements

(Metaphorical Rhetoric) wrestled with, parade of wrestled with parade of mountain of labyrinth of tangled

    P14
the bills do not require, as they should, that the results of these studies be used

(Counter-factual Modals) as they should ought to failed to missed opportunity short of

    P15
Congress needs to find the courage to allow Medicare to pay preferentially

(Ethical Appeal) find the courage to find the courage duty to imperative bold step test of will

AUTHOR ATTITUDE ·

Argumentative essays 中,作者如果要 support 某个 position,会用 explicit positive markers 来 signal approval——'good news' 不仅是 positive,而且 emotionally engaging,表明作者不是 neutral observer 而是 active supporter,同时引用 prestigious journal 增加 credibility

在 balanced argumentation 中,提及 opposing views 是必要的,但关键是作者如何 handle them——如果作者 immediately provide counterarguments 或 conditions that mitigate concerns,说明他的 underlying attitude 是 defensive of the original position,这区分了 genuinely balanced analysis 和 superficially balanced advocacy

Prescriptive modals 在 academic writing 中 signal author's normative stance——当作者频繁使用 'should/ought to/needs to' 而非 'could/might/may',说明他不是 neutrally presenting options 而是 pushing for specific actions,这是 advocacy 而非 pure analysis 的 hallmark

Sophisticated argumentation 会在 advocating 的同时 acknowledge limitations——如果作者在 expressing support 时仍然用 'no one knows', 'hard to know how well', 'might not',说明他的 attitude 不是 unrealistic enthusiasm 而是 informed optimism,这种 nuanced position 是 academic credibility 的 marker

  6. Argumentative essay 的 structural choices reflect author's purpose——如果 bulk of content 是 detailed explanation of solutions 加上 rebuttal of critics,而不是 exposing flaws,说明 author's communicative intent 是 persuade readers to support,这个 structural analysis 能 override 个别 ambiguous sentences 的 misleading signals

  • good news P2

这个 phrase 是 explicit positive evaluation marker,作者用它来 frame subsequent information favorably——在 argumentative writing 中,直接使用 'good news' 表明作者不仅在 report facts 而且在 guide reader's emotional response,这是 persuasive strategy 的 clear signal,表明 supportive attitude

同类词 encouraging development positive sign promising fortunate welcome news

  • should P2

Modal verb 'should' 表达 author's normative judgment——作者在说 reforms 'should help' 时,不是 stating fact 而是 making recommendation,这个 prescriptive modal 表明他在 endorse 这些 measures,认为它们是 desirable course of action,这是 advocacy 而非 neutral analysis

同类词 ought to needs to must it is essential that it is imperative that

  • Critics say P5

这个 phrase 引入 opposing viewpoint,但关键在于作者的 subsequent response——作者用 'That is far less likely to happen if' 来 immediately counter the criticism,这个 pattern (introduce criticism → quickly refute) 表明作者在 strategically present counterarguments 只是为了 dismiss them,从而 strengthen 他对 original position 的 support

Critics argue Some claim Opponents contend Skeptics worry Detractors point out

  • no one has an easy fix P1

这个 phrase 看似 negative,但在 argumentative context 中它是 rhetorical strategy——作者 acknowledge difficulty 是为了 establish credibility 和 realistic expectations,紧接着的 'likely to be achieved' 表明他不是 using difficulty as reason to abandon effort,而是 framing realistic optimism,这种 balanced presentation 增强 persuasiveness

同类词 no easy fix no simple solution complex challenge not straightforward difficult to solve

  • The final legislation should P6

这个 prescriptive structure 表明作者在 actively recommend specific policy features——'should' 在这里不是 polite suggestion 而是 strong recommendation,作者在告诉 lawmakers 什么 ought to be done,这是 advocacy writing 的 characteristic feature,表明他在 push for particular outcomes

同类词 should include must incorporate ought to adopt needs to feature is essential to have

  • simplification ought to save money P7

'Ought to' 是 normative modal,表达 author's expectation based on logical reasoning——作者在说 simplification 'ought to' save money 时,不是 merely describing possibility 而是 stating what logically should happen,这反映他对 reform benefits 的 confident expectation,支持 optimistic attitude

同类词 ought to should is expected to will likely is positioned to

  • Congress needs to find the courage P15

这是 strong prescriptive language 结合 emotional appeal——'needs to' 是 obligation modal,'courage' 是 emotionally charged word,暗示当前 inaction 是 cowardice——这个 combination 表明作者不仅 support policy 而且 frustrated with current situation,在 urge action with urgency

同类词 needs to must has to it is imperative is essential

  • no one knows how P9

这个 hedging phrase 表面上是 expressing uncertainty,但在 context 中它 serves to acknowledge limitations honestly——作者在 numerous positive claims 中插入这些 qualifiers,表明他的 optimism is realistic 而非 blind,这种 intellectual honesty 增强 credibility 而不是 undermine support

同类词 no one knows it is unclear uncertain hard to predict difficult to determine

  • we suspect that spending would come down P15

First person plural 'we' 加 epistemic verb 'suspect' 表明 author's personal commitment——作者在 making personal prediction,用 'we' 把自己 include in the judgment,这比 impersonal 'it may happen' 更强 in commitment level,表明 invested belief in positive outcome

同类词 we suspect we believe we think in our view from our perspective

Title 'REFORM AND MEDICAL COSTS' 已经暗示要讨论 reform 的 effectiveness。第1段 开篇就说 'Americans are deeply concerned',然后提出 'The answer is...',这是典型的 problem-solution pattern 加上 author's position,不是客观的 exposition 或 factual reporting。

第1段 最后一句 'The fundamental fix... is likely to be achieved only through trial and error and incremental gains' 就是 central claim,表达 author's judgment about solution。这不是 explaining how something works(说明文),也不是 reporting what happened(新闻),而是 arguing what approach is necessary。

第2段 开头 'The good news is...' 明确引入 supporting evidence(House and Senate bills)来 back up 第1段 的 claim。第3段 提供 explanation of causes('propelled by two things')来 build foundation for later arguments。第4段 用 'Here are some of the important proposals' 来 preview specific supporting points。这是 classic argumentation structure:thesis → support → explanation → detailed evidence。

第5段 中 'Critics say Congress will give in to lobbyists...' 然后立即反驳 'That is far less likely to happen if...'。第14段 开头 'Critics have charged...' 后面用括号 '(That would be true only if...)' 来 dismiss the criticism。这是 argumentation 的 hallmark:anticipate objections and defend position,而不是 neutrally presenting information 或 reporting what critics said。

全文充满 prescriptive statements:第2段 'should help slow',第6段 'The final legislation should',第10段 'to urge providers to adopt reforms',第15段 'Congress needs to find the courage',第16段 'We believe negotiation could work'。这些 directive language 表明 author 在 advocating for specific actions,不是 explaining how system works 或 reporting what officials decided。

每个 section 都在 build case for reform effectiveness,不是 neutrally informing 或 objectively reporting。
